HIPPA & Notice of Privacy Practices

Your privacy is important. As a healthcare provider, I am required by law to protect the privacy of your protected health information (PHI) and to provide you with notice of how your information is used and safeguarded.

Information shared during counseling sessions is confidential and will not be disclosed without your written authorization, except as required or permitted by law. These exceptions may include:

  • Risk of harm to yourself or others

  • Suspected abuse or neglect of a child, elder, or vulnerable adult

  • Court orders or other legal requirements

Electronic records are maintained using HIPAA-compliant systems with appropriate administrative, technical, and physical safeguards.

A full Notice of Privacy Practices is available upon request and will be provided prior to the start of services.

In case of emergency

Telehealth counseling is not designed to provide emergency services.

If you are experiencing a mental health emergency, are in immediate danger, or feel unable to keep yourself safe, please:

  • Call 911 or your local emergency number, or

  • Go to the nearest emergency room

  • Local crisis services in your area

Emergency resources will be reviewed during the intake process, and clients are encouraged to keep local emergency contacts readily available.

Informed Consent and Telehealth terms.

I provide counseling services exclusively via secure telehealth platforms. Telehealth allows clients to receive professional mental health care from the privacy and convenience of their own location, using secure video technology that complies with HIPAA standards.

I am licensed in Ohio, Massachusetts, Arizona, and Minnesota, In accordance with state law, clients must be physically located in one of these states at the time of each session. If you are traveling or relocate outside of these states, services may need to be paused until you are physically present in a state where I am licensed.

Telehealth counseling involves the use of electronic communications to enable mental health services when the client and clinician are in different locations. Telehealth has benefits, including increased accessibility and convenience, but also carries potential risks such as technology failures or limitations in communication.

By engaging in telehealth services, clients acknowledge that:

  • Telehealth is not appropriate for all mental health concerns

  • Confidentiality applies to telehealth services, with the same legal and ethical limits as in-person counseling

  • Technology disruptions may occur despite reasonable safeguards

  • Telehealth is not a substitute for emergency or crisis services

A full telehealth informed consent document will be reviewed and signed prior to the start of services.
